Heart of Hope Asian American Hospice Care is a community-based, minority-run non-profit 501(c)(3) Medicare accredited medical organization that has been serving the entire Santa Clara County since 2012. Due to the cultural and language barriers, Asian-Americans have been underserved when facing severe illness and end-of-life care issues. We have assumed the pioneer role to bridge the gap and hope to promote the health equity to serve the underserved Asian-American community in the San Francisco South Bay area with our multilingual and multicultural staff including English, Chinese Mandarin, Cantonese, Taiwanese, Vietnamese, Korean, Hindu, Malay, Punjabi, Farsi, and Tagalog.
We believe no end-of-life patient should be turned away from hospice services due to a lack of sufficient medical insurance or capability to pay. The organization has offered charity care to 6 end-of-life hospice patients in 2023. We set out to carry on our mission to cherish life and care for the patients and their families during the toughest yet most precious time of the patients’ lives with our full medical, psychosocial, and spiritual care and support from the date they are admitted till they pass away and following by free grief care to their bereaved love ones for another 13 months. Regardless of their insurance coverage, we treat them as we would like to be treated when we are in similar circumstances. The organization also offers supplemental small respite grant for families in financial hardship who are also caring for end-of-life.
Since 2012, in the process of building up Heart of Hope Asian American Hospice Care, many challenges and obstacles have been overcome owing to the trust, encouragement and support from all over the community. This has assured us that we would never be alone when we strive to serve the community with love, professionalism and compassion! With the continuing support from the community, we believe love can flourish and hope can be sustained in the most challenging time for the end-of- life patients and their families.
Our Free Community Services
Besides Hospice Care , the organization also provide free community services as follows:
Home visiting volunteers for critically ill patients
A national toll-free telephone hotline providing emotional support to critically ill patients and their family members
Free nationwide support group
Free nationwide hospice and health education webinars
Counseling on issues related to end-of-life patient care and advance directives
Provide psychological counseling and bereavement counseling to critically ill and terminally ill patients and their families
Consultation on hospice-related issues and referrals to hospice organizations/community resources
A life was changed
by the work of Heart of Hope Asian American Hospice Care ...
I lost my beloved mother, my best friend in 2017, and during her 5-month illness I was very panicked and couldn’t imagine how to live without her. I was almost vanished in the ocean of deep grief. She was sent home from the hospital to receive hospice care from Heart of Hope (HOH) and died the next day. Within the two days of care, the family-like care for my mom and me helped me quickly build a sense of trust. HOH also provides one year of free grief counseling for the bereaved family members. I was particularly touched by the CEO, a senior therapist. She understood that mother's death had impact me profoundly and that recovery from psychological trauma would be more difficult than ordinary people, so she provided me with more intensive free face-to-face counseling on top of her heavy management duties. Her professionalism, kindness, and genuine care helped me to acknowledge and accept my grief and find a way to express my emotions. Followed by her help, a weekly grief support telecare volunteer assigned to me for another 8 months, and I gradually developed a new understanding of the resilience of life and can walk steadily with this sorrow and no longer be so afraid of the future. Thank you, Heart of Hope for walking me through the toughest life journey with unconditional love and patience.
